All the individual forms needed for the 2011-12 ANSAA Annual Report are listed below along with detailed instructions. We are trying to make this as user friendly as possible and hope the following format will simplify the process. If you have trouble with the forms or you do not have Microsoft Word, contact the ANSAA office (501-803-3888) for more detailed instructions. For more information on filling out the forms, check the ANSAA Newsletter attached with your Aug. e-mail from the ANSAA office. 2011-12 Forms (all in Microsoft Word format): All 2011-12 forms have some deletions and/or additions. Please do not alter the forms to fit a different format and make sure all forms used have a 2011-12 header. ANSAA ACCREDITED SCHOOL FORMS: Annual Meeting Registration Form* New Administrator Orientation Registration Form* New Administrator Qualification Form* Application for Accreditation Renewal*
NON-ACCREDITED MEMBER SCHOOL FORMS: Annual Report - Non Accredited Associate Members* Annual Meeting Registration Form* Application for Initial Accreditation* INSTRUCTIONS: All 2011-12 forms have some deletions and/or additions. Please do not alter the forms to fit a different format and make sure all forms used have a 2011-12 header. Each form is in Microsoft Word format. Click on the link above and select "Save" and then save it to a place that you can easily locate (i.e. Desktop, My Documents). NOTE: You cannot select "Open" as the first step as the file needs to be saved on your hard drive to work properly. When you are ready to complete the forms, go to Microsoft Word and “Open” your “Saved” ANSAA file. When the forms are completed, save the files for your own records with the appropriate name of the report (i.e. School Name-Annual Membership 08-09.doc). This will allow you to recall the information for next year’s annual report.
